• This form requires uploading a copy of your license to practice, and State-issued ID. Please have digital copies ready when you begin the form. If you do not hold a current license to practice, please use the Basic Volunteer Interest Form instead.
  
  

• Our current clinic times determine our ability to place you for volunteer work. You’ll be asked which of these you’re available for (if any) on the form.

  Tuesdays, 9:00am - 4:00pm (Ypsilanti)
  Tuesdays, 9:00am - 2:00pm (Wayne Clinic)
  Wednesdays, 1:00pm - 4:00pm (Ypsilanti)
  Thursdays, 9:00am - 4:00pm (Ypsilanti)
  Fridays, 9:00am - 2:00pm (Wayne Clinic)
  1st & 2nd Saturdays, 8:30am - 12:00pm (Wayne Clinic)
  2nd & 4th Saturdays, 9:00am - 12:00pm (Ypsilanti)
  Rotating 3rd Saturdays, Neurology & Sleep Clinics 9:00am - 12:00pm (Ypsilanti)
  
  Some admin roles can be filled outside of these clinical hours.

  If you are not available during times when we have clinics, we still want to hear from you, but we may not be able to place you.

  
   

• Due to the level of training required, we respectfully require a 1yr commitment to volunteer. If you cannot commit to a year, we’d still love to hear from you, but we may not be able to place you.

Note: We are unable to facilitate observation hours in our clinics.
NPs: Require a minimum of one year practical NP experience unless a prior agreement has been made with an existing clinician volunteering at Hope Clinic. NPs looking for preceptors: all of our clinicians are volunteers and we do not have any preceptor opportunities unless already pre-arranged with a clinician that is volunteering at our medical clinic.
